Chronic Disease Management Programs and Diabetes Self-Management Program (DSMP)
The Coalition began offering DSMP in October 2010. This is a comprehensive six week program that covers a variety of topics to help individuals gain self-management tools. Classes are held at a variety of locations throughout the community and are available for anyone to take, whether they currently have diabetes or not.
Topics covered during the six weeks include:

Problem-Solving Health Care System Communication Treatment Decisions

Chronic Disease Self-Management Program (CDSMP)
This program will now replace other Coalition programs that were taught in the past (Healthy Hearts, Healthy Lifestyles, etc.).
Any individuals that currently have high blood pressure, high cholesterol, a chronic disease, or are overweight will be put into this program. Program topics include:

Problem-Solving
Health Care Professionals Future Health Plans
Health Care System Depression
Communication Medications
Treatment Decisions
Worksite Wellness
Successful worksite wellness programs have also been provided by the Coalition. Partnering with area organizations, the Coalition designs a wellness program that benefits both the employee and the employer. Worksite wellness programs have been designed to include the following components: screenings, personal goals, education classes, aerobics, monitoring system, and incentive items. Worksite Wellness opportunities are endless and can be designed for any length of time (weekly/monthly) based on the needs and desires of those participating.
