Diabetes Self Management is designed to:
Help individuals that have been diagnosed with diabetes or prediabetes know how to manage their blood sugar through diet, exercise, and possibly medication.
- Give participants the information and encouragement that they need to prevent prediabetes from progressing to diabetes, or to manage their diabetes.

Chronic Disease Self Management Program (CDSMP)
For those who have one or more of the following conditions:
- High Blood Pressure/Heart Disease
- Headaches
- Cancer
- Arthritis
- Asthma/ Lung disease
- Back Pain
- Depression
- Any other chronic health condition
Chronic Disease Self Management is designed to help participants:
- Manage symptoms
- Get started with healthy eating and physical activity
- Communicate effectively with your health care team
